Superior Senior Care is Medicaid Certified by the State of Arkansas for in-home community waiver programs for low-income residents. We can help you with the application to see if you qualify.
ELDERCHOICES
Elderchoices is an Arkansas Medicaid home and community-based waiver program designed for individuals 65 and older, who due to physical, cognitive or medical reasons, require a level of assistance that would normally have to be provided in a nursing facility. The major goal of this program is to provide services that assist eligible persons to remain in their homes or live with family in order to prevent or delay institutionalization.
Superior Senior Care is certified to provide caregiver referrals for the following Elderchoices programs:
- Homemaker — provides basic upkeep and management of the home. Services include laundry, shopping, errands, simple household tasks and meal preparation.
- Chore — includes heavy cleaning and/or yard maintenance ONLY in extreme, specific circumstances, when lack of these services would make the home uninhabitable.
- Respite — provides temporary relief to a primary caregiver who is providing long-term care for individuals in their homes. It may be provided outside the client’s home to meet an emergency need or as periodic scheduled relief.
- Adult Companion — companions may assist or supervise clients with tasks, such as meal preparation, laundry, light housekeeping, bathing, eating, dressing and personal hygiene when these services are required in accordance with a therapeutic goal.
AAPD is a Medicaid Waiver program that provides home and community-based services to adults between the ages of 21 and 64 with physical disabilities.
- Attendant Care: Assistance in accomplishing tasks of daily living based on need and approved by a physician. Based on need, a client may receive up to 8 hours a day, 7 days a week of attendant care. Clients must be able to direct and supervise their attendants.
- Case Management and Counseling Support: Counseling Support Managers referred by Superior Senior Care will provide orientation to the concept of consumer-direction and training on how to recruit, interview, hire, evaluate, supervise, manage or dismiss attendants. Counseling Support Managers provide support services to all AAPD waiver clients, monitoring their needs and assisting the client in continuing to live as independently as possible.
PERSONAL CARE
Personal care services referred by Superior Senior Care are primarily based on the assessed physical dependency need for “hands-on” services with daily living activities. These include: eating, bathing, dressing, personal hygiene, toileting and ambulating. Hands-on assistance in at least one of these areas is required to qualify for Personal Care under Medicaid. This type of assistance is provided by a personal care aide based on a beneficiary’s physical dependency needs (as opposed to purely housekeeping services). The tasks the aide performs are similar to those that a nurse’s aide would normally perform if the beneficiary were in a hospital or nursing facility.
TARGETED CASE MANAGEMENT
These services are provided by a health-care professional referred by Superior Senior Care and include the assessment of abilities and needs, coordination of services, and on-going monitoring of the condition of the client as well as the quality of services being delivered by the various other providers.
